REFERRAL FORM
If you are an attorney, give the form to your provider
and request the image come through CP Healthcare.
If you are a provider, simply fill out the order on our
referral form and send it to CP Healthcare via fax or email.
Once we receive the order, CP Healthcare works to contact the patient/client and get them
scheduled within 24 hours. Based on the patient/client’s address, we schedule them to the
closest network facility to their home. CP Healthcare understands the importance of getting the
image completed quickly so the provider can begin proper treatment and the attorney can
build his/her case.
